Coordinates social services within this innovative senior center and from the community. Provides supervision to case workers and social work students, as well as direct services to clients.
Consults with and keeps the director informed of needs and problems related to social services, as well as of the on-going services provided.
Supervises case workers in daily operations of case assistance, including assigning cases, coordinating, problem solving, client relations, and resolving emergent issues.
Assesses the case workers’ skills and identifies specific training needs for staff.
Conducts weekly social service meeting with case studies and common-problem solving, as well as discussion of staff morale related issues.
Reviews case records to ensure that services are provided according to the assessed care plan.
Assists in resolving difficult client issues.
Assures that social service staff adheres to mandates established by DFTA contract and that quality services are provided, including appropriate referrals, case coordination and follow ups.
Ensures and tracks progresses of unit fulfillments of all monthly contractual requirements.
Submits monthly statistics related to social services to the director in a timely basis.
Attends case work related trainings in Selfhelp and in DFTA.
Attends community based or DFTA meetings surrounding social service coordination issues.
Supervises social work interns according to their school requirements.
Conducts the monthly mental health education forum, and focus groups as needed.
Facilitates the monthly grandparent support group sponsored by DFTA.
Conducts the weekly older immigrant support group.Organizes and conducts depression screening, memory screening and other mental health screenings as needed.
Cooperates with the community nurse and the program coordinator to promote Kiosk, Dakim, and Age Tastic programs. Cooperates with the program coordinator to promote the Virtual Senior Center.
Organizes social actions/advocacy as needed.
Coordinates social services provided by community resources, like Assemblywoman Nily Rozic’s weekly mobile office, NYLAG’s monthly mobile service, HR’s monthly Food Stamps service, etc. by making referrals and providing follow-ups.
Participates in planning for improving services by interpreting social factors pertinent to development of program.
