Vitalis/Care Connections, our national team of NursePractitioners, provides in home health services for Medicare, Marketplace, andMedicaid recipients.
Through our program we are able to meetour patients where they live and conduct health assessments in the privacy andcomfort of their home.
By utilizing Epic, our customized electronichealth record, as well as advanced point-of-care testing equipment, we are ableto provide our patients with a thorough and comprehensive health careexperience. We use technology to strategically schedule visits with the aim ofmaximizing patient care time while decreasing the time spent driving from hometo home. Visits vary from preventive care for diabetics, to post-partumassessments, and annual comprehensive exams to make sure our health plansreceive a complete and accurate picture of our patients health conditions andneeds.

ABOUTUS\\: Molina Healthcare, a FORTUNE 500, managed care organization,arranges for the delivery of health care services and offers health informationmanagement solutions to nearly five million individuals and families whoreceive their care through Medicaid, Medicare and other government-fundedprograms in fifteen states.
AS ANURSE PRACTITIONER YOU WILL\\:
Complete a comprehensive review and assessmentof patients medical history, current medications, psycho-social well-being andsocial determinants of health
Document progress notes that support comprehensivecapture of all active diagnoses
Provide incomplete preventive careservices as determined by HEDIS or Medicare 5 Star requirements
Perform point-of-care lab anddiagnostic testing, such as Hemoglobin A1C, monofilament, nephropathy screeningand diabetic retinal exams
Follow-up on abnormal results andcoordinate ongoing care with PCP
Write prescriptions with the supportand oversight of a collaborating physician
Help at-risk patients transition fromthe hospital and other facilities to their own home through in-personassessments, post discharge medication reconciliation, referrals and carecoordination with the PCP
Qualification: Job Qualifications
MINIMUM REQUIREMENTS\\:
Boardcertified as an Advanced Nurse Practitioner in the specialty area of familymedicine
Holdan active, unrestricted state Nurse Practitioner license, as well as DEA andNPI number
Havea current provider card in Basic Life Support (BLS)
Comfortablewalking up flights of stairs while carrying up to 50 lbs. of equipment, as someresidences may not have elevators
IDEALCANDIDATES HAVE\\:
Apassion to serve the underserved
Previousexperience working with Medicaid, Marketplace and Medicare populations
Atleast 3 years of clinical experience preferably in home health, palliative careand/or hospice settings
EpicEHR experience
Bilingualor multi-lingual communication skills
