Direct Care Position: Immediate Start Flexible Hours.
$10.00 to $11.00 Depending upon experience
Creative Foundations’ Supportive Living Program allows individuals with disabilities to live as independent as possible in the community. Creative Foundations is seeking Team Members to provide direct care support to individuals with Developmental Disabilities.

Must Meet Minimum Qualifications:
- Must have a current Driver’s License
- Proof of High School Diploma or G.E.D.
- Must be at least 18 years old.
- Satisfactory driving record from the Ohio BMV with no more than 5 points or citations over 9 miles an hour.
- Satisfactory criminal background check.
- Proof of current automobile insurance listing applicant as a driver on the policy with state minimum liability amounts.
- Reliable automobile to transport individuals
